2. `T_recovery_recognized` set **only** when both `T_cross_obs` and `T_confirm` exist (EOS unrecovered → recognition NULL).
3. case_I c5 tautology removed.
4. `refractory_polls`: only value `1` supported (assert); do not imply other values are tested.
